1.0 General Information
a) We reserve the right to change these terms and conditions at any time.
b) These Terms and Conditions apply to The Pelvic Studio (www.thepelvicstudio.com)
c) This website is intended to provide informative and educational content, but does not supersede professional advice. It is general in nature and is not intended as specific advice for your health and wellbeing. Diagnosis and treatment plans for health-related issues should always be made in consultation with an appropriate health professional.
2.0 Physiotherapy Consultations
2.1 Booking Terms & Information
a) Physiotherapy consults can be booked online using the online booking system. Alternatively, patients can call or email The Pelvic Studio.
b) Patients that book a telehealth consult are required to pre-pay for their consult using our online STRIPE payment system, all other physiotherapy appointments are payed for post appointment via EFTPOS. Our classes are to be pre-paid prior to the commencement of our class term via direct bank transfer.
c) Patients are required to claim their private health rebate (if applicable) on-line or in person with their health insurer. It is up to the patients to work out this process and to make the claim themselves.
2.2 Physiotherapy Consultation Rescheduling/Cancellation Policy
a) If you cannot make your physiotherapy consultation you must use the Online Booking System or call/email The Pelvic Studio within 24 hours of your appointment time to avoid a 50% cancellation fee. In regards to our classes, once the class term commences your class term is non-refundable
2.3 Access to the Online Booking System
Anyone not complying with the above Terms & Conditions may have their access to the online booking system removed.
3.0 Exercise Rehabilitation/Group Sessions
3.1 Physio Assessment
For all new exercise class participants we recommend a physiotherapy assessment prior to undertaking exercise rehabilitation. This ensures The Pelvic Studio is aware of your current physical state including any relevant injuries or health issues. The Pelvic Studio can use the information obtained in the assessment to tailor/modify exercises to ensure you get the best outcome from your exercise rehab and reduce the risk of injury. If you wish to claim your exercise rehabilitation through a health fund an annual physio assessment is mandatory.
3.2 Booking Terms & Information
a) Exercise Rehabilitation are to be booked via phone/email, 1:1 and 2:1 exercise rehab session are payed for post appointment, all pregnancy and postnatal class packages must be pre-paid for via direct bank transfer.
b) Our classes run in 6-week terms. A 6 week class pack must be pre-paid to secure a spot in one of our class terms.
3.3 Exercise Rehabilitation Cancellations
a) If you cannot make your exercise rehab session (1:1 & 2:1 sessions) we ask that you call/email us >24 hours from your scheduled appointment time to avoid a 50% cancellation fee.
b) If you cannot make your pregnancy or postnatal class, this class is ultimately forfeited and no credit or refund is offered.
4.0 Privacy
The Pelvic Studio employees have a responsibility to maintain the privacy of personal health information and related financial information. The privacy of this information is every client’s right.
This policy outlines how the practice handles personal information collected (including health information) and how the security of this information is protected. A privacy statement is made available to clients and anyone who requests it.
There are no degrees of privacy. All client information, including the information of employees who may be clients, must be considered private and confidential, even that which is seen or heard. Therefore, such information is not to be disclosed to family, friends, employees or others without the client’s approval. Sometimes details about a client’s medical history or other contextual information, such as details of an appointment, can identify them - even if no name is attached to that information. This is still considered health information and it must be protected. Client information may not be disclosed either verbally, in writing, in electronic form, or by copying either at the practice or outside it, during or outside work hours, except for strictly approved use within the client care context, or as legally directed.
4.1 Privacy Statement
This statement informs clients how their health information will be used. This includes the sharing of information to other organisations to which the practice usually discloses client health information, and any law that requires the particular information to be collected. Client consent to the handling and sharing of health information should be provided at an early stage in the process of client care. Clients should be made aware of the collection statement when giving consent to share health information.
In general, quality improvement or audit activities for the purpose of seeking to improve the delivery of a particular treatment or service is considered a directly related secondary purpose for information use or disclosure. Specific consent for this use of client health information is not required.
4.2 Storage of Health Records
We will create and keep a health record for you which will record all relevant information relating to the treatment provided to you. We may also collect and store health information about you from third parties such as your doctor. Your health records are managed and protected in accordance with the applicable state and federal laws. Health records are legally required to be stored for a number of years following the conclusion of your treatment with us. You are also able to request access to a copy of your Health Record. Please discuss this with your practitioner or our practice manager should you wish to do this at any time.
